Promotes preventive health care, provides assistance and acts as an advocate for clients enrolled in the EPSDT Medicaid Program or Child Health Plan Plus Programs. Educates Medicaid recipients about the benefits of preventive health care and available services; determines if barriers to obtaining health care exist and develops a plan to eliminate barriers; arranges or refers for needed services. Maintains an assigned caseload of clients; assists, monitors and documents client progress using standard data based software, coordinates with other agencies for a full range of services.
What You Bring/Job Requirements
In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
1 year related experience
Bilingual capabilities: Spanish/English preferred
Efficient knowledge of the local and state Department of Social Services programs including Medicaid eligibility guidelines and benefits
HS diploma or GED required
B. A./B.S. in social or health related field or two years college course work in social work or a health related fieldpreferred
A valid State of Colorado driver's license required
M-F 8am -12:00 pm
**This is a grant funded position only assured until June 30, 2021; hiring team will try for more funding, but there is absolutely no guarantees
Sedentary work -(prolonged periods of sitting and exert up to 10lbs force occasionally)
