Connections Health Solutions is a physician-owned, multifaceted behavioral health company whose mission is to address any behavioral health need at any time. We are hiring Licensed Practical Nurses (LPN) for new substance abuse clinics who will be responsible for client care and dispensing controlled substances, Methadone, Buprenorphine, and Suboxone to clients based on physician's orders following careful assessments of the clients. Under general supervision of the Clinical Director and Nursing Coordinator, provides support to case management and medical staff; and provide services in accordance with clients' individual treatment plan that results in a positive behavioral change

  • Participate in interdisciplinary staffing's for clients as scheduled and/or as needed
  • Coordinate with primary care physicians as appropriate and facilitate communication with providers as requested by attending
  • Observe, record, and report a client's reaction and responses to medications as appropriate
  • Count/measure and dispense Methadone, Suboxone, or Buprenorphine to clinic clients as directed by physician's orders
  • Follow best practices and requirements in accordance with SAMHSA and DEA Opioid Treatment Program guidelines
  • Follow the Code of Federal Regulations regarding opioid diversion measures
  • Follow the CSAT guidelines regarding take home dose criteria
  • Ability to explain the purpose of toxicology screens and outcomes
  • Follow the pregnancy protocol as needed
  • Conduct weekly calibration of dosing machine
  • Use and understand the central registration system
  • Follow client dosing protocols
  • Review and monitor opioid medications per DEA guidelines
  • Participate in and initiate diversion control measures
  • Daily review of all opioid activity and reports
  • Knowledge of community resources for the opioid and SMI population
  • Maintains all required certifications and trainings

  • EDUCATION: Graduation from an accredited Nursing program
  • EXPERIENCE: At least 2 years' behavioral health related nursing work experience; experience with chemically dependent client population preferred.
  • LICENSE/CERTIFICATION: Licensed as a Practical Nurse in Arizona (LPN)
  • LANGUAGE REQUIREMENTS: None, bilingual a plus
    • Knowledge of DSM, Title 36 and basic case management skills
    • Knowledge of different types of medications commonly prescribed for psychiatric, personality and substance abuse disorders.
