Under general supervision, provides direct and indirect, covered services and treatment to youth and child individual clients and their families, coordinates community supports and stakeholder referral need, using the Child and Family Team Process, as part of the behavioral health program of Centro De La Familia-Phoenix Outpatient Clinic. Responsible for managing appointment schedule, assigned caseload as a clinical liaison, conducting assessments, implementing treatment plans, facilitating out-of-office services, performing crisis services when needed, and participate in ongoing learning experiences and treatment team consultation and coordination.

- Masters degree in a behavioral health related field and/or 2 years of full time behavioral health work experience
- Knowledge and understanding in general mental health, substance abuse, co-occurring disorders, and serious mental illness.
- Familiarity with application of behavioral principles in youth treatment, understanding of the Arizona Child and Family Team model.
- Possess a valid driver’s license and ability to be insured under automobile insurance policy.
- Able to work a flexible schedule as needed per program requirements.
- Must successfully complete the agency’s orientation and demonstrate competence regarding skills and knowledge per R9-20-204.
- Have basic typing/computer skills.
- Have excellent verbal and written communication skills.
- Have good organization and time management skills.
- Ability to obtain fingerprint clearance card, CPR and First Aid.
- Ability to attend required Child and Family Team training within one month of assuming position and participate in ongoing learning requirements.
- Bilingual language skills in English/Spanish and as appropriate to population served preferred.
- Must be 21 years of age.
A.) Performs comprehensive intake and ongoing assessments.
- Accurate documents the assessment service, including assessment tools and progress notation.
- Assessment information includes individual and family voice and participation, informed consent, and integrated into initial and ongoing treatment planning.
- Conducts ongoing assessment, documents functional and symptomatic improvement, cultural values, crisis planning, functional family assessment, and addenda.
B.) Works with individual and families that present with a range of behavioral health presentations, including co-occurring disorders.
- Recognize symptoms to the full and broad range of DSM diagnoses.
- Ability to provide routine, urgent, and emergent interventions.
- Application and integration of American Society of Addiction Medicine criteria in service planning and implementation.
- Application and integration of the Child and Adolescent Service Intensity Instrument.
- Ability to apply motivational enhancement, stages of change, family systems, and appropriate family communication process.
- Ability to understand and screen for serious mental illness, and complete requests for determination.
C.) Educate families on the stages of child development.
- Ability to demonstrate education and verbalization of information in a culturally competent and sensitive manner.
- Working knowledge of 0-5 and continuing lifespan development.
D.) Facilitate and coordinate the child and family team process.
- Able to promote an individual/family culture.
- Effectively engage families in process.
- Knowledge of crisis management concepts and practices and apply and exercise such knowledge and skills as necessary to develop crisis and safety plans.
- Facilitate and documents strengths, needs and cultural discovery.
- Collaborates, coordinates, and consults with other involved service providers and treatment team members.

E.) Implementation and application of behavioral management strategies in therapeutic related activities.
- Understands benefits and principles of behavioral management strategies.
- Model and assist in the implementation and integration of age-appropriate behavioral strategies.
F.) Manage and monitor individually assigned caseloads.
- Maintains personal contact with client/families and not exceeding 30 days.
- Accurately documents progress notes same day as service, not to exceed 24 hours; crisis documentation completed same day as service.
- Follow-up to service commitments, engagement, re-engagement for missed appointments, letters of intent, welfare checks, and appropriate discharge functions completed within required time periods.
- Maintains and monitors caseload statistical information, including presenting issues, length of stay, and outcome measures when assigned.
G.) Provide individual/family counseling and are able to facilitate groups. Engage consumers, their families, and their community supports in treatment, provide illness self-management education, and coordinate services with referral sources and discharge planning.
- Coordinates and conducts group functions and facilitates cohesiveness in its entirety that demonstrate improvement in a client’s/family’s symptom and functioning level.
- Conducts home visits, other community-based services, as needed and requested by client and/or family.
- Demonstrates ability to de-escalate and de-brief disruptions to group and /or family content and process.
H.) Assist in working with clients with medication and case management services.
- Consults and coordinates with prescriber, prescriber liaison, laboratories, medical providers, pharmacies, and assists with eligibility and access to prescription services when needed.
- Consults on initial and ongoing psychiatric evaluation requests.
I.) Responsible for maintaining and coordinating service plan development and updates, when applicable.
- Demonstrates integration of the 12 principles in the development of the service plan.
- Integrate needs identified in assessment with service plan development and review.
- Evidence of child participation in service plan development.
- Evidence of family/stakeholder participation in service plan development.
- Accurate and timely completion of service plan/review.
J.) Performs Clinical Record Reviews.
- Understanding of all licensing, regulatory and agency guidelines.
- Document significant information in each file in a clear and timely manner.
- Provides appropriate feedback.
K.) Achieve assigned weekly/monthly productivity expectations.
- Know and match individually assigned productivity value amounts on a weekly basis, as set for the position, throughout the fiscal year.
- Monitor appropriate productivity needs, variables, and opportunities.
L). To be available for shift coverage and crisis resolution as needed.
1. To provide on call coverage for crisis situations as needed.
2. To share in weekend and holiday coverage as scheduled.
3. To coordinate additional community treatment services, as needed.
M). Perform other duties as assigned.
Reports to the Youth and Adult Services Clinical Manager /Supervisor of Centro De La Familia Outpatient Clinic.
Oversees day-to-day clinical operations as clinical liaison for assigned caseload and serves as lead for assigned child & family team members, for the Youth Services outpatient behavioral health program of Centro De La Familia-Maricopa outpatient clinic. Responsible for sharing rotation of crisis lead for outpatient clinic during business hours, and on-call crisis cell phone when required through Provider Network Organization.
Effective oral and written communication skills, including; ability to read and comprehend technical instructions, short correspondence, memos, business correspondence and mental health related documents, ability to write clear, concise and accurate correspondence. Capable of establishing positive interpersonal relationships with a broad range of people.
College level mathematical skills required.
Ability to solve complex problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or diagram form. Demonstrated skill with problem solving techniques.
Ability to operate a personal computer with office software, printer, copy and fax machines, calculator, and other office equipment.
Ability to become licensed with the State of Arizona Board of Behavioral Health Examiners as LMSW, LAC, or LAMFT; LCSW, LPC, or LMFT preferred and based upon program requirements. Valid Driver’s License and proof of insurance meeting agency standards required. First Aid, CPR, and Fingerprint Clearance Card required.
