Job Description

Home Start, a non-profit agency dedicated to the prevention of child abuse and strengthening of families is seeking a Case Manager for the Maternity Housing Program (MSP). The Case Manager will provide in-home and center-based case management and supportive services to the MHP residents and be responsible for overseeing day-to-day activities for clients and volunteers.  The MHP is a residential facility for pregnant/parenting young women ages 18-24.

Duties and responsibilities:

  1. Conducts client assessments consistent with program requirements.
  2. Primary coverage for THP clients (weekly sessions and notes, monthly progress notes, tracking of client progress on case plan goals, intakes and discharge documentation)
  3. Provides case management services including: identification of client’s needs and developing client’s Individual Case Plan Goals with ongoing monitoring and follow-up on progress towards those goals.
  4. Provides services to clients including, but not limited to supportive counseling, crisis intervention, mentoring, and parent education.
  5. Completes weekly groups with residents based on parenting education, life skills, and resident meetings. Oversees the supervision of childcare during the groups. Provides engaging and enriching curriculum for residents based upon all different learning styles and education level.
  6. Tracks client progress and case notes in ETO
  7. Communicates with the County
  8. Prepares and submits monthly caseload reports, monthly invoices, Quarterly reports
  9. Teaches and demonstrates goal setting and decision making.
  10. Demonstrate familiarity and acts in accordance, with established safety regulations, policies and procedures.
  11. Provides support to families in identifying and obtaining community resources.
  12. Maintains a flexible working schedule to accommodate resident’s schedules and needs.
  13. Ensures accurate and timely collection and reporting of data to contract specification for evaluation through the maintaining of monthly reporting, data entry and timely case records documenting all services provided
  14. Monitors participants’ progress in developing skills needed to assure positive outcomes and communicating areas of need to all supportive staff.
  15. Assures security of confidential information and materials.
  16. Attends agency, program and collaborative meetings as assigned.
  17. Attends and participates in training as appropriate.
  18. Participate in weekly individualized supervision with supervisor which focuses on areas of responsibility and boundaries, discussion of the THP residents and their progress in the program, any quality or performance issues, problem solving, and individual skill building.
  19. Participate in weekly Housing Team Meetings which focuses on identifying challenges, barriers and effective techniques in providing services to participants and achieving outcomes, coordination of MSP services with other programs.
  20. Performs other duties as assigned.


Skill / Requirements

Education:  A minimum of a Bachelor’s degree or equivalent in social work, psychology, education and/or related human services field required.  Demonstrated experience providing intensive home-based case management with a working knowledge of early child development, family systems, community resources, substance abuse treatment and interventions required.  Experience working with low income or homeless populations and pregnant and/or parenting women in a shelter or residential setting is preferred. Abilities:  Is trauma-informed and able to treat residents with understanding of previous homeless history, possible CPS involvement, and challenging backgrounds.  Demonstrated ability to be culturally sensitive and respect diversity.   Able to provide support, structure, empathy, sound judgment, insight into human behavior and family relations. Skills: Effective communication skills.  Must have the ability to work in a team environment, using a team dynamic approach for best practices with MHP residents.  Must be able to receive constructive feedback and utilize motivational interviewing techniques to work with all residents. Computer  literate as appropriate.  Bilingual in Spanish, Asian, or a Pacific language preferred. Other Requirements: Must have a valid California driver’s license, insurance and a reliable vehicle and be willing to use it on the job.
