This position will complete crisis risk assessments in the local medical hospital and make recommendations for inpatient psychiatric treatment or follow up outpatient care. In addition to assessing patients who present in crisis at the local ER and medical inpatient unit, they will also make a referrals when appropriate to the Medication Assistance Treatment (MAT) programs at Horizon Health and Wellness (HHW). This position will provide education and information to the community about our Crisis behavioral health, outpatient treatment programs and MAT program.

Requirements

Master degree or Doctorate plus State Licensure as an independent practitioner, i.e, LPC, LMFT, LCSW, LISAC.

Must be at least 21 years of age.

One year Supervisory experience and/or program development.

Possess valid Arizona driver’s license with acceptable driving record, home telephone, and reliable transportation.

Clinical supervision of crisis staff, crisis intervention services to agency participants and residents in the community.

Appropriate referrals and placements within the treatment continuum.

Knowledge of crisis program development.

Demonstrate ability to provide counseling/case management to SMI participants in the Mental Health program.

Demonstrates knowledge of special populations such as substance abuse.

Demonstrates ability to develop treatment plan and summary based on participants needs.

Demonstrates ability to distinguish between alcohol and other drug-related symptoms and other pre-existing physical problems or pathologic behaviors.

Demonstrates ability to understand the bio psychosocial influences and effects of dependencies.

Demonstrates knowledge of how the acute and chronic nature of the dependence and the ways medical, emotional, behavioral, social, educational, and other interventions are used to treat the participant.

Demonstrated knowledge of referral resources available to participants.

Ability to make independent decisions.

Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.

Will provide on-call after hours supervision/support when needed.

Qualifications Preferred

Bilingual – English/Spanish.

Experience working in Integrated Healthcare, Behavioral Health or Primary Care environment.

Knowledge of catchment area gained through residence in local community.
