Other information:
1. Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; or
2. Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by a MSW as noted above and
a. Has one year of social work experience in a health care setting or
b. Has a baccalaureate degree from a school of social work accredited by the Council on Social Work Education, is employed by the hospice before December 2, 2008, and is not required to be supervised by an MSW.
3. Licensed as a social worker in the State of New Mexico

Summary:
Responsible for assessing developing, and implementing appropriate social work care plans which take into account the psychosocial, financial, community resource, long term planning and bereavement needs of hospice patients and their families. Functions as a key member of the interdisciplinary healthcare team
