Cares for patients in the home environment, by providing primary care and urgent medical care through our House Calls, Complete Care and Hospital at Home programs. These serves patients have usually been referred due to their medical fragility and complicated conditions. They and family members will need well-coordinated medical care concordant with their personal goals for care. Provider will lead and work collaboratively with a team, including nurses, social workers, case managers, therapists , aides, and clerical staff to provide care to patients. The position also requires regular communication with other PCP s and specialists about ongoing care and works in collaboration with our Palliative and Hospice providers to offer those services to appropriate home- bound patients.
*Provides comprehensive medical care to patients with acute and chronic illnesses.
*Interacts with the House Calls, Hospital at Home and Complete Care teams to establish a plan of care. Participates in Palliative Outpatient IDT meetings to integrate patient care.
*Provides real time feedback to other PCPs and referring physicians regarding clinical status and follow-up plans to optimize patient care and clinical efficiency.
*Serves as a medical resource to interdisciplinary team members.
*Offers clinical oversight of the work of the Hospital at Home nurses.
*Provides medical care five/days/week; assures appropriate coverage and response after hours and on weekends. Participates in on call coverage.
*Documents all patient encounters and communications appropriately
*Provides clinical input into the development and review of appropriate clinical protocols and processes.
*Meets measurable program goals in clinical, quality, financial and compliance realms.
*Attends and participates in requested meetings and planning aligned with key program strategies and goals. Meet and maintain all provider credentialing and QI standards. Provides full and appropriate documentation of direct patient care services to assure good patient care and support successful billing for all services.
*Enhances professional growth and development through participation in educational programs, literature review, in-services, and workshops.
*Performs other functions as required.
Type of Opportunity: Part Time (.45 to .89)
Work Schedule: Days
M.D., board eligible or certified in Internal Medicine, Family Medicine or other applicable board certified practice. State of New Mexico Medical License, DEA, NM Pharmacy Certificate. Prefer experience in primary care. Rural health care experience an asset.
