|Working Title||Community Health Worker / Intensive Case Manager|
Under indirect supervision, CHWs/ICMs work closely with Pathways Navigators, County Substance Abuse Program staff, City of Albuquerque Family & Community Services staff, community health and behavioral health providers, and numerous other social services agencies to provide Intensive case management and care coordination for persons in treatment for addiction, persons with behavioral health disorders, persons with co-occurring disorders, and persons recently released from the Bernalillo County Detention Center. CHWs will be employed by UNM, but housed the majority of their time at different sites in the community, included, but not limited to: County Metropolitan Assessment & Treatment Services facility (MATS), First Nations Community Healthsource, First Choice Community Healthcare, the County-funded Re-Entry Resource Center located downtown, Heading Home Westside Shelter, or other potential sites.
Duties and Responsibilities:
1. Works long-term with individuals with addiction and/or behavioral health disorders on the social determinants of health, addressing multiple issues simultaneously, or over a longer period of time.
2. Helps assess most critical needs of clients (individuals mentioned in #1) and takes measures to address these needs while working with individuals on a long-term plan of action.
3. Assures that clients are enrolled in Medicaid, SNAP, and other public benefits that they may be eligible for.
4. Facilitates communication and coordination of services between providers and clients.
5. Connects clients to community resources, including but not limited to: legal assistance, housing, food, clothing, education, life skills classes and employment.
6. Coordinates and monitor services, including comprehensive tracking of client’s commitment and compliance in relation to agreed upon plan of action.
7. Prepare periodic reports on outcomes achieved by clients as well as on barriers encountered.
8. Maintains comprehensive and confidential files on clients, which include notes, release of information, assessments and other documents acquired on behalf of the individual.
9. Works collaboratively and effectively within a team (CHWs/therapists/social workers/navigators) and establishes positive, supportive relationships with co-workers.
10. Travels regularly to community locations, various agencies, and other outreach destinations. This position will require that selected individual work at least two evenings/nights at the Westside Shelter in addition to follow-up outreach in the community for accepted ICM clients.
11. Performs miscellaneous job-related duties as assigned.
|Conditions of Employment|
High school diploma or GED; at least 3 years of experience directly related to the duties and responsibilities specified.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
· Prior experience working with populations affected by addiction, behavioral health disorders, homelessness, discrimination, persons that have experienced violence either as a victim or a perpetrator, etc.
· Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Prior practice with motivational interviewing skills helpful.
· Knowledge of community agencies and resources.
· Ability to plan, implement and evaluate individual client plans of action.
· Knowledge of barriers associated with addiction, behavioral health disorders, as well as socio-economic barriers that may be encountered by client.
· Skill in use of personal computers and related software applications, including e-mail.
· Ability to set professional boundaries and stick to them.
· Strong organizational skills and ability to establish priorities.
· Bilingual/bicultural (Spanish-English and Diné-English preferred)
